Reporting Requirements

The Administration for Community Living (ACL) is announcing an opportunity for public comment on the proposed collection of certain information by the agency. ACL plans to continue an existing approved collection of information for semi-annual and final reports pursuant to the requirements of its discretionary grant programs. Under the Paperwork Reduction Act of 1995 (the PRA), Federal agencies are required to publish notice in the Federal Register concerning each proposed collection of information, including each proposed extension of an existing collection of information, and to allow 60 days for public comment in response to the notice. This notice solicits comments on the information collection requirements relating to the continuation of an existing collection for Performance Progress Reports previously approved for discretionary grants funded by the U.S. Administration on Aging (AoA), which is now a part of ACL. The Updated Guidelines for Preparing Performance Reports (doc) provide instructions for semi-annual and final performance reporting pursuant to requirements of ACL discretionary grant programs.

AoA has created new guidelines for reporting for grantees that have received funding through the Title IV Discretionary Grants program of the Older Americans Act. These Guidelines provide instructions for semi-annual and final performance reporting pursuant to requirements in the OAA. To view the instructions, read the Current Guidelines for Preparing Performance Reports for Grants (doc). While each program area may have individual reporting requirements, both financial and programmatic reporting is required by statute for all AoA grant programs.

As of February 1, 2011, the Department of Health and Human Services will begin the transition from use of the SF-269, Financial Status Report (Short Form or Long Form) to the use of the SF-425 for expenditure reporting. Complete all lines as appropriate except lines 10. a through c. SF-269s will no longer be accepted for expenditure reports due after that date. If an SF-269 is submitted, AoA will return it and require the recipient to complete the SF-425.

The SF-425 form is available from the links below.
