FY12 American Indian/Alaska Native Health Disparities Program

The purpose of the American Indian/Alaska Native Health Disparities Grant Program is to improve the effectiveness of efforts to eliminate health disparities for American Indian and Alaska Native communities. Through this FY 2012 announcement, the OMH is supporting utilization of epidemiological data to identify high priority health status objectives and to make recommendations relative to the services and/or activities required to address those objectives; development of alliances and partnerships to improve coordination of and access to quality health services; and development of programs designed to increase the representation of the American Indians/Alaska Natives in the health care workforce (including research positions).

FY12 Eliminating Lupus Health Disparities Initiative (ELHDI)

The purpose of the ELHDI is to eliminate lupus related health disparities among racial and ethnic populations disproportionately affected by this disease. This overall purpose will be achieved by disseminating a national lupus education program to health professionals and student trainees in targeted schools and health care systems and practices. One grantee will be funded to conduct this national initiative.

FY12 HIV/AIDS Health Improvement for the Re-entry Population Demonstration Program (HIRE)

The HIRE Program seeks to improve the HIV/AIDS health outcomes of formerly incarcerated persons re-entering the mainstream population by supporting community-based efforts to ensure their successful transition from state or federal incarceration back to their communities.

FY12 Peer-to-Peer Outreach Model to Curb HIV/AIDS Transmission Among High Risk Minority Youth and Adolescents (CHAT)

As the lead agency to improve and protect the health of racial and ethnic minority populations through the development of health policies and programs that will help eliminate health disparities, the Office of Minority Health, through the mechanism of the CHAT Program, seeks to improve the HIV/AIDS health outcomes of high risk minority youth (13-19 years old) by supporting community-based efforts to increase HIV/AIDS prevention/education efforts, testing, counseling and referrals. The CHAT Program will integrate HIV testing, peer educators/lay health workers, counseling and linkages and social media and technology into a wide- ranging community model to reduce the risk of infection in high prevalence communities. This project will also encourage partnerships among traditional service providers targeting high risk adolescents, such as substance abuse prevention centers for youth; foster care agencies working with youth; youth serving organizations; youth runaway shelters; and peer-topeer education programs.

FY12 Youth Empowerment Program (YEP)

The purpose of the YEP is to address unhealthy behaviors in minority at-risk youth and provide them opportunities to learn skills and gain experiences that contribute to more positive lifestyles and enhance their capacity to make healthier life choices. The YEP is intended to test the effectiveness of innovative approaches in promoting healthy behaviors among minority youth at-risk for poor health/life outcomes. These demonstration grants require a multi-partner approach involving institutions of higher education, primary and secondary schools, community organizations and institutions, and the community at-large.

Women's Successful Transition and Reentry into the Community from Prison - Request for Proposals (RFP)

Funding is available for the planning and implementation of three (3) demonstration projects to enhance reentry programs' existing reentry services with comprehensive, gender-responsive, and trauma-informed approaches that support women's effective transition from prison to the community. Please note that ongoing operations will not be funded through this RFP. Funding is specifically for enhanced activities rather than usual operations.
In addition, process and outcome evaluation activities are a requirement for the awarded demonstration sites along with at least one paper for publication. Technical Assistance will be provided to support the awarded organizations with evaluation activities.
Demonstration sites that are selected for funding will each receive $25,000 for a planning period of approximately four months (May 21, 2012 – September 28, 2012). After successful completion of the planning period, demonstration sites will each receive an additional $25,000 per year for two years (October 1, 2012 – September 30, 2013 and October 1, 2013 – September 30, 2014).
