I-90, Application to Replace Permanent Resident Card

Purpose of Form :
Use this form to replace a "Green Card." (You may also file this form online.) Do not submit this form if you are a conditional resident seeking to remove conditions. Submit Form I-751 to remove conditions on a Green Card obtained through marriage or Form I-829 to remove conditions on a Green Card obtained through financial investment in a U.S. business.

Application Type B: My card was issued but never received.

  • There is no fee, but you must meet these criteria:
    • We mailed the card over 30 days ago
    • It was returned to us as undeliverable
    • You have not moved from the address you provided us
  • You must submit a copy of the Form I-797, Notice of Action, for Form I-485, Form I-751, Form I-829, or Form I-90 that resulted in the issuance of your card.
  • You must also submit a copy of government-issued identification that contains your name, date of birth, photograph and signature.

 Application Type D: My card contains incorrect information due to USCIS error. 

  • There is no fee
  • You must send us the original card that contains the incorrect information
  • You must also send evidence documenting your correct name or biographical information

 Changes to Submitted Applications

If it is necessary to change any information on your submitted I-90 application, please bring evidence documenting the change with you to your in-person appearance at your local Application Support Center (ASC). Do not send written correspondence regarding changes.

Applicants younger than 14 years of age and 80 years and older

You must submit the Biometric Service Fee and attend the scheduled ASC appointment.  A Green Card cannot be produced without a photo.
