Contractor Template

Contractors should use this template to provide information to the contract-level Contracting Officer Representative (i.e., Project Officer) to initiate the badging process. This requirement applies to contractor and subcontractor employees requiring access to EPA facilities or EPA information systems. If there is a requirement for contractor or subcontractor employees to have on-site access for at least 24 hours a week for at least 6 months a year they will be required to undergo a background investigation in order to receive an EPA Personnel Access and Security System (EPASS) badge.

The template was developed to assist in providing a uniform format for the transmission of the required contractor employee information. The template contains drop down menus when entering data in various data cells, i.e. Employee Type, Program Office, Work City and State, Birth State, Birth Country, Citizenship, Previous Investigation and Investigative Agency. IMPORTANT NOTE: The template must include contractor employees’ full, "formal" names as found on employment records, Social Security cards, driver’s license, etc. Incomplete, inaccurate names and/or nicknames are UNACCEPTABLE and will delay badge issuance.
