Weekly Reports of Fatalities, Catastrophes, and Other Events
Description
This table provides links to weekly summaries of fatalities and catastrophes resulting in the hospitalization of three or more workers. Employers must report these incidents to OSHA within eight hours. The summaries below include only preliminary information, as reported to OSHA Area Offices or to States which operate OSHA-approved State Plans. OSHA investigates all work-related fatalities and catastrophes. Once an OSHA investigation is complete, the summary report will be updated with a webpage link to the corresponding inspection, which will list citation information.

Fatalities and Catastrophes Report Archives (FY09, FY10 and FY11)
Weekly summaries of fatalities and catastrophes reported in FY09, FY10 and FY11 are available on the Fatalities and Catastrophes Archive page.
OSHA Standard for Reporting of Fatalities and Catastrophes:
OSHA Standard, 29 CFR Subpart 1904.39, Reporting Fatality, Injury and Illness Information to the Government requires that within eight (8) hours after the work-related death of any employee or the in-patient hospitalization of three or more employees, employers must orally report the fatality/multiple hospitalization by telephone or in person to the OSHA Area Office or to the State Plan Office that is nearest to the site of the incident. Employers may also use the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742).
