DUA - OASIS
Effective June 1, 2012, all DUA request submissions must follow the e-mail Subject line instructions located on the DUA-Forms page (use the link in the left hand menu).
The Outcome and Assessment Information Set (OASIS) is a group of data elements that represent core items of a comprehensive assessment for an adult home care patient and form the basis for measuring patient outcomes for purposes of outcome-based quality improvement (OBQI). This assessment is performed on every patient receiving services of Home Health agencies that are approved to participate in the Medicare and/or Medicaid programs.
Researchers must contact the ResDAC for free assistance in requesting CMS data. Requests submitted directly to CMS will not be accepted. Contact ResDAC via:

States and U.S. Territories may obtain CMS data by completing the following documentation:
- Written Letter of Request
- Data Use Agreement (DUA) form
States and U.S. Territories may receive assistance and submit their request to the CMS Regional Office representative Regional Office representative serving your area.
