504 Authorization Wizard

W504v2009.1.exe and W504v2009.1W7.exe-- the 504 Wizard Setup Program.

The 504 Wizard Setup Program will install the Wizard, Boilerplate and sample documents on your workstation.

Tip -- If you are not interested in downloading and installing the Wizard now, but just want to view or print the Boilerplate, click here.

Requirements

• A computer running under Windows 95, 98, 2000, XP, Vista or NT.   Windows 7 has not been tested, but if Word functions, the Wizards should also.

• Microsoft Word 2000 (9), Word 2002 (10), Word 2003 (11) or Word 2007 (12)  to run the 504 Wizard (note compatibility issues above)

• The Wizard has been reissued to provide support for Word 2007 (12) -- download W504v2009.1w7.zip.

• The free Adobe Acrobat Reader to view the Boilerplate.

Installation Instructions

Note: The 504 Wizard requires long file name support. Do not install it on a network that does not support long file names.

1. Click  here to download W504v2009.1.exe - W3 -The zipped 504 Wizard Program and Setup files for Word 2000 (9), Word 2002 (10) and Word 2003  -- Not compatible with Word 2007. 

2.  

   OR -- Click  here to download W504v2009.1W7.exe (File size: approx. 73 MB) -W7 - W504v2009.1w7.zip - The zipped 504 Wizard Program and Setup files for Word 2007 (12) -- Not compatible with Word 2003.

3. Save the zip file to your Windows Desktop

4. Unzip the file W504v2009.1.exe or W504v2009.1W7.exe to your Windows Desktop

5. Locate W504v2009.1.exe or W504v2009.1W7.exe and double-click on the file.

6. Follow the instructions on the screen.

7. When the installation is complete, you can delete W504v2009.1.exe or W504v2009.1W7.exe from your desktop.

To start the Wizard after it has been installed:

1. Start Microsoft Word

2. In the File menu, select New

3. Double-click on 504 Wizard 2009.1.dot or 504 Wizard 2009.1.dotm

To view the Boilerplate after the wizard has been installed:

Note: You need the free Adobe Acrobat Reader to view the Boilerplate.

1. Start the Wizard (see above)

2. In the 504 Wizard menu, select National 504 Authorization Boilerplate

About the Opinion of CDC Counsel

The standardized text for the Opinion of CDC Counsel required by the Authorization is available in Appendix D to the Boilerplate. Additionally, a MS-Word file containing a copy of Appendix D (Filename: D504v2009.1.doc) was copied to your MS-Word document directory by the 504 Wizard Setup Program.

If you cannot locate D504v2009.1.doc on your workstation, download it from the  504 Authorization Library.

To open D504v2009.1.doc:

1. In MS-Word, select Open from the File menu

2. Browse to the directory containing D504v2009.1.doc

3. Double-click on D504v2009.1.doc
