CAPLines Authorization Wizard

WCAP2012W3.exe and WCAP2012W7.exe-- the CAPLines Wizard Setup Programs

The CAPLines Wizard Setup Program will install the Wizard, Boilerplate and sample documents on your workstation.  The W3 version is designed for Word 2003 and prior versions of Word.  The W7 version is designed for Word 2007 and above.  Please note that the W3 version will be discontinued 90 days from release date.

Tip -- If you are not interested in downloading and installing the Wizard now, but just want to view or print the Boilerplate, click here.

Requirements

• A computer running under Windows 2000, Vista, 7

• Microsoft Word 2002 (10) or Word 2003 (11) for the W3 CAPLines Wizard and the W7 CAPLines Wizard for Word 2007 (12) and Word 2010 (14)

• The free Adobe Acrobat Reader to view the Boilerplate.

Installation Instructions

Note: The CAPLines Wizard requires long file name support. Do not install it on a network that does not support long file names. Note CAUTION below.

1. Click here to download WCAP2012W3.zip or WCAP2012W7.Zip (File size: approx. 8 MB).  The "exe" startup files must be extracted before running.

2. When prompted, save WCAP2012W3.zip or WCAP2012W7.zip on your Windows Desktop

3. Unzip WCAP2012W3.exe or WCAP2012W7 to your desktop.  In the Windows START menu, select RUN

4. Type WCAP2012W3.exe or WCAP2012W7.exe and press Enter, or double-click the file name on your Desktop.

5. Follow the instructions on the screen.

6. When the installation is complete, you can delete WCAP2012W3.exe and WCAP2012W3.zip or WCAP2012W7.exe and WCAP2012W7.zip from your desktop.

To start the Wizard after it has been installed:

1. Start Microsoft Word

2. In the File menu, select New

3. Double-click on CAPLines Wizard 2012.dot or CAPLines Wizard 2012.dotm

To view the Boilerplate after the wizard has been installed:

Note: You need the free Adobe Acrobat Reader to view the Boilerplate.

1. Start the Wizard (see Above )

2. In the CAPLines Wizard menu, select National CAPLines Authorization Boilerplate

About the Opinion of CDC Counsel:

The standardized text for the Opinion of CDC Counsel required by the Authorization is available in Appendix D to the Boilerplate. Additionaly, a MS-Word file containing a copy of Appendix D (Filename: DCAP2012.doc) was copied to your MS-Word document directory by the CAPLines Wizard Setup Program.

If you cannot locate DCAP2012.doc on your workstation, download it from the CAPLines Authorization File Library.

To open DCAP2012.doc:

1. In MS-Word, select Open from the File menu

2. Browse to the directory containing DCAP2012.doc

3. Double-click on DCAP2012.doc
