Submit a Request

The Privacy Act permits you to access records about yourself that are maintained by the U.S. EPA in a Privacy Act system of records, unless the records are covered by an exemption. In addition, you may request that incorrect or incomplete information be changed or amended.

Privacy Act requests must be submitted in writing to the Privacy Act Officer in accordance with instructions in the Privacy Act notice. Your request must include sufficient information to allow verification of your identity. If you cannot determine the applicable system of records, you may write to the EPA Privacy Act Officer.

The Privacy Act Officer may require you to submit a signed and notarized statement indicating that you are the individual to whom the records pertain, and that you understand it is a misdemeanor punishable by a fine up to $5,000 to knowingly and willfully seek or obtain records about another individual under false pretenses.

At this time, all Privacy Act requests must be submitted in writing to:
