Service Coordination

Applicable Authorities

There are several ways to include service coordination in an MLTSS program.  You can continue to provide it in MLTSS using the authority you have used in your traditional program, which may include:

  • Defining it as an HCBS state plan service under section 1915(i)
  • Including it as a a section 1915(c) waiver service or administrative activity
  • Electing targeted case management as an optional state plan service

If you choose to include consumer directed services in your program under 1915(c), 1915(i), 1915(j) or 1915(k), you will need to make available a support “broker” to beneficiaries who elect consumer directed services. The primary focus of a support broker/counselor is to provide counseling and to act as the beneficiaries’ agent as the beneficiary considers his/her plan and approach to available supports. Alternatively, if you want to enhance service coordination beyond what is provided in your traditional program, or if you do not cover service coordination in your traditional program, you may work with your MLTSS contractors to offer service coordination as an additional “value added” service.
