Q. What if my reference is out of town and cannot complete the form by the 14–day deadline?

A. Exceptions will be made for those with extenuating circumstances.


Q. How will I get the reference form to my references?

A. You will need to download this Reference Form(Download PDF file. 78 KB) and email it to your references. They will need to come back to this site to submit, by going to the "Reference Submission" page. The link to this website is provided on the form. You could provide them with the link as well: References.


Q. How do I ensure that my reference submits the form before the 14–day deadline?

A. It is the applicant's responsibility to remind and follow-up with their references. Please note that applications will only be reviewed once the NHLBI has received all documents listed below:

  1. Application Form
  2. Reference Forms
  3. A CV that includes all previous and current employment, with start and end dates, your title, and a list of your accomplishments, honors and publications
  4. A 1–2 page statement describing why you are interested in the internship program and how you meet the selection criteria.

A. Applications are rolling, but once you start the application process you have 14 days to submit the necessary documents.
