Linking Clinical Practices and Community Organizations for Prevention

Background and Introduction

The Agency for Healthcare Research and Quality has established a goal of improving care along the continuum from primary prevention to chronic care management. This includes health promotion, disease prevention, health maintenance, and the diagnosis and treatment of chronic illnesses. The Agency seeks to explore how partnerships among primary care and medical specialty providers, the community, and public health organizations are developed, strengthened, and sustained to improve care and meet the needs of patients and families.

Innovations Exchange Resources

This special Innovations Exchange page was created to facilitate the ongoing collaborative work of interested stakeholders and Summit participants. New Innovation Profiles and QualityTools were developed based on the case studies of promising linkages presented at the 2008 Summit and 2010 Summit. The Innovations Exchange Web site offers additional innovations and tools that promote healthy behavior through services delivered in clinical, public health, and community-based organizations.

Event
AHRQ's Health Care Innovations Exchange held a Webcast titled Vermont Blueprint for Health: Working Together for Better Care on September 25, 2012.

This event featured a video presentation about successful linkages among primary care, public health, and clinical community resources in the state of Vermont, followed by a panel discussion. Materials from the event are available here.
