Narrator: Here’s another health tip from AHRQ, the U.S. Agency for Healthcare Research and Quality.

If you’re scheduled for a hospital stay, here are some ways to reduce your risk of medical errors. Speak up if you have questions or concerns. Don’t assume that your care team has all the information they need about you. When you’re leaving the hospital, get a plan for what you should do at home including what medicines to take and when you need a follow-up visit with your regular clinician.
