Self-Assessment Certification Template (Word)


As outlined in the FLETA Standards and Procedures Manual (2010 Edition), applicants seeking academy and/or program accreditation must conduct a self-assessment. If non-compliance is identified during the self-assessment, the agency will follow the corrective action plan process as outlined in the FLETA Standards and Procedures Manual. Applicants will complete the attached certification template and submit it to the Office of Accreditation for review and approval to proceed with the FLETA Assessment. The certification letter must be signed by the Authorizing Official and submitted 60 days prior to the scheduled FLETA Assessment.
