Ralph Folsom was responsible for the overall methodology and estimation for the model-based Bayes estimates and prediction intervals. Akhil Vaish, Neeraja Sathe, and Kathryn Spagnola were responsible for the estimation and analysis. Bill Wheaton and Justine Allpress were responsible for the mapping. Mary Ellen Marsden provided substantive review. Kathy Spagnola and Brenda Porter generated the tables. Neeraja Sathe provided oversight for production of the report. Richard S. Straw and Richard Hair edited the report, and Joyce Clay-Brooks formatted its text and tables. Web files were prepared by Pamela Couch Prevatt, Teresa F. Gurley, and Kim Cone at RTI and Beatrice A.
