The New Hampshire Office of Community and Public Health, located within the Department of Health and Human Services, is the state's public health agency with responsibility for assuring the health and well-being of communities and populations in New Hampshire by protecting and promoting the physical, mental, and environmental health of its citizens and by preventing disease, injury and disability. The Office is organized into five major program divisions as follows: Family and Community Health, Community Support, Chronic Disease Prevention and Health Promotion, Epidemiology and Vital Statistics, and Health Services Planning and Review.
