Classroom / Other Scheduling Training Request Form

To request use of on-site classrooms or meeting facilities at the FLETC, Cheltenham, Maryland facility, please complete ONE scheduling request form for each SEPARATE event, date or time slot you are requesting. 

Submit scheduling request form by one of the following methods:

  1. electronically by clicking on the "submit" button below; or

  2. print and fax completed form to the Cheltenham Scheduling Office at 301-877-8521

PLEASE NOTE: Scheduling requests are considered tentative and are not definitively scheduled or confirmed through submission of this form.  You will receive e-mail notification to:  (1) confirm your request; OR (2) notify you that we were unable to accommodate your request.

NOTE: with the exception of Reimbursable Agreements, all other payment options require the submittal of student rosters and financial documents IN ADVANCE of the training date. If documents are not received prior to training date, training will be CANCELLED.
