What Is Transition Planning

Transition planning is a process for youth with disabilities that focuses on working toward results-oriented goals after high school, such as post-secondary education, vocational rehabilitation, integrated employment or independent living. It also helps youth develop life skills through hands-on experiences, so they can become successful, contributing members of society.
