Using the Online Request Form

If you would like to submit a request or appeal to OIP through this online request form you will be able to set up a personal account. You will be asked to provide your name and contact information and to create a unique username and password for this site. Your unique user account will allow you to log into this site and track any request or appeal that you have made to OIP. By submitting a request or appeal through this online request form, OIP will then communicate with you through your online account. OIP's determination, as well as any responsive documents, will be provided to you electronically, through your secure, online account.

When OIP first receives your request or appeal, your account will be updated with a holding number until OIP's staff assigns your request or appeal an individualized tracking number. You will also receive an electronic copy of OIP's acknowledgement letter. As OIP processes your request or appeal, OIP's staff will update the status of your request or appeal in your account. You can access your account at any time to check the status of your request or appeal. When an interim or final determination is made in response to your request or appeal, OIP will post an electronic copy of its response letter and any responsive documents in your account.

If you prefer not to use this capability you may still submit your request or appeal to OIP by mail, fax, or e-mail.

  • Submit and Track a Privacy Act Request

    To protect your privacy, when you request records about yourself you are required to verify your identify. This system does not accept verification of identity materials. If you use this system you need to supplement your online request. There are multiple ways to do this.

    You may mail, fax or email:

    A signed Form DOJ-361.

    Include within your request your full name, current address, and date and place of birth and then having your signature notarized

    Include within your request your full name, current address, and date and place of birth and sign the request and include the following words: "I declare under penalty of perjury."

    Once your verification of identity is received we will begin to process your request. You will then be able to check the status of your request and receive any responsive documents through your account.

  • Submit and Track an Administrative Appeal

    OIP processes administrative appeals from any adverse determination made pursuant to the FOIA and Privacy Act for all Department of Justice components except the United States Parole Commission.

    When submitting an administrative appeal to OIP, you will be asked to identify the component and provide the initial request number for the determination that you are appealing.
