CDC Awards Community Transformation Grants
Implementing and expanding prevention programs proven to work provides our nation with the opportunity to improve the health of Americans and control health care spending.
On September 27, the Department of Health and Human Services announced approximately $103 million in Community Transformation Grants (CTG) to states and communities, serving approximately 120 million Americans throughout the country. These projects are part of a comprehensive HHS initiative to address the health and well-being of our communities, and will be administered by the Centers for Disease Control and Prevention.
The awards to 61 states and communities will support the planning and implementation of projects proven to reduce chronic diseases, which are responsible for 75% of health care costs in the United States. By promoting healthy lifestyles, especially among population groups experiencing the greatest burden of chronic disease, these grants will help improve health, reduce health disparities, and control health care spending. These awards will be dispersed among state and local government agencies, tribes and territories, and state and local non-profit organizations.
Awardees will address the following priority areas:
- tobacco-free living;
- active living and healthy eating; and
- evidence-based quality clinical and other preventive services with a central focus on prevention and control of high blood pressure and high cholesterol.
Reducing health disparities and advancing health equity are core principles of the CTG program. Awardees will target underserved populations, and at least 20 percent of the grants will be distributed among rural and frontier areas. All communities will work on both population-wide improvement and reducing disparities in health outcomes across identified population groups.
Among the 61 CTG communities, 35 communities and states are receiving funding for the implementation of evidence-based and practice-based interventions designed to improve health and wellness, and 26 communities and states are receiving funding for capacity building to improve community health by laying a solid foundation for community prevention efforts.
