Quality and Innovation Learning Network

The Learning Network is a community of sites from the IPC 1, IPC 2, and IPC 3 Collaboratives. They have achieved the core elements of the patient-centered medical home and are working to expand quality improvement changes across their entire sites.

The Learning Network, the third IPC program component, is where IPC 1, IPC 2, and IPC 3 teams continue their improvement work after participation in the IPC collaboratives. The teams are building a learning community equipped to improve quality and access to care across the entire Indian health system. In the Learning Network, IPC sites continue sharing and working with other sites on improvement activities. The Network offers:

  • common change packages and measurement sets;
  • opportunities to work in depth on a specific improvement activity;
  • opportunities to become innovative in the way care is delivered with like-minded facilities; and
  • opportunities to mentor other sites to improve patient care across the Indian health system.

The Learning Network also offers didactic materials on complex topics, such as: advanced access systems; nurse-family partnerships; home health monitoring for severely, chronically ill patients; elder care assessment and management; self-management support systems; and behavioral health integration into primary care practice.

Learning Network activities are facilitated with faculty support to meet the needs of IPC team staff members and leadership. Learning Network members meet with local experts and national faculty throughout the year. National or regional learning session calls are held at least once a month.
