Through IPC, we are learning together how to build the system of care American Indian and Alaska Native people deserve. We are demonstrating progress and seeing results by using quality improvement models developed by our IPC teams and experts in health care quality improvement.
The IPC Care Model is based on the Chronic Care Model developed by the MacColl Institute for Health Care Innovation, leaders in the field of quality improvement. Their model was adopted by the World Health Organization and has been tested and implemented widely in the United States and other countries.
As IPC sites worked with the Chronic Care Model, they identified ways to reframe it to reflect the unique features of health care in the Indian health system. They also adapted it to address the strong role of family and the need to fully integrate the community and the Tribes into the vision for health care. IPC’s changes to the MacColl Institute’s Chronic Care Model align closely with concepts of the medical home in the health care literature and rely on the full optimization of the IHS clinical information system.
