The Improving Patient Care (IPC) 4 Collaborative is working to improve health and promote wellness for American Indians and Alaska Natives and to support the four IHS priorities. It’s a pathway toward a redesigned system of care that is grounded in the values and culture of the community served. The IPC 4 Collaborative focuses on strengthening the positive relationships between the health care system/team and the individual, family, and community. The Care Model for the Indian health system serves as a framework to guide the creation of a patient-centered medical home, an accessible and patient-centered system of care that provides safe, timely, effective, efficient, and equitable care. Thirty-three sites participate in the IPC 4 Collaborative, including 14 IHS, 13 Tribal, and 6 Urban Indian facilities.
The Quality and Innovation Learning Network is also working to improve health and promote wellness for American Indians and Alaska Natives across all ages. The Learning Network is an active learning and innovation community that provides continued support for IHS, Tribal, and Urban Indian programs. The Network aims to achieve the following key changes of the patient-centered medical home: care centered on the patient and family; tribe and community; care team; access and relationship; and culture of quality. Sixty-three sites participate in the Quality and Innovation Learning Network, including 37 IHS, 19 Tribal, and 7 Urban Indian facilities.
