State Demonstrations to Integrate Care for Dual Eligible Individuals – Design Contracts

 

Partnering with States to Coordinate Care for Dual Eligibles

Under the State Demonstrations to Integrate Care for Dual Eligible Individuals, fifteen states across the country have been selected to design new approaches to better coordinate care for dual eligible individuals. 

CMS will provide funding and technical assistance to states to develop person-centered approaches to coordinate care across primary, acute, behavioral health and long-term supports and services for dual eligible individuals.  The goal is to identify and validate delivery system and payment coordination models that can be tested and replicated in other states. 

CMS is also making technical assistance available to all states interested in improving services for dual eligible individuals.

 

States Selected to Received Design Contracts

The states selected to receive design contracts are California, Colorado, Connecticut, Massachusetts, Michigan, Minnesota, New York, North Carolina, Oklahoma, Oregon, South Carolina, Tennessee, Vermont, Washington, and Wisconsin. Each state will be awarded up $1 million to develop a model describing how the state would structure and implement its planned intervention. 

States that engage with beneficiaries and other stakeholders and successfully complete their design contract may be eligible to receive support to implement their proposals.  After federal review of the proposals, CMS will work with states to implement the plans that hold the most promise.

Selection for a design contract does not constitute federal approval or endorsement for any implementation activities associated with these plans.

The Medicare/Medicaid Coordination Office and the Center for Medicare and Medicaid Innovation (Innovation Center) are partnering to bring this opportunity to states . More information about the Innovation Center can be found at http://innovations.cms.gov/.  The Medicare/Medicaid Coordination Office and the Innovation Center will work closely with the Center for Medicare (CM), the Center for Medicaid, CHIP and Survey & Certification (CMCS) and other partners across the Centers for Medicare and Medicaid Services and the Department of Health and Human Services(HHS) on this initiative.
