    Departmental Forms Management

    Reviews, clears, numbers, and controls inventory for all official HHS forms, and ensures compliance with policies including Section 508 of the Americans with Disabilities Act.

The Departmental Forms Management Program team reviews, clears, numbers, and controls inventory for all official forms used within HHS. In addition, the team ensures that forms comply with policies including Section 508 of the Americans with Disabilities Act.

The team provides the form clearances required and/or mandated by the Office of Management and Budget, Chief Information Officer, Information Technology Service Center, Freedom of Information Act (privacy), and Records Management offices.

The Forms Management Program can give customers the ability to migrate data from Section 508 compliant HHS forms to a secure centralized database. In addition, the team can create customized user interface screens and reporting tools to help manage, search and analyze their data.

Services offered include:

  • Developing HHS-wide initiatives, policy, and procedures governing the program
  • Providing technical assistance and training to Operating Division (OpDiv) Forms Management Officers
  • Section 508 compliance testing and remediation
  • Forms data translation and migration to a secure database for report generation and storage management

Performance:

95% of customer requests to create, modify, or delete Departmental forms will be processed within three (3) business days of receipt. Customer will receive an email confirmation.

Rate:

$10.00 per FTE per year
