Reporting Guidance
Interim Progress Report
(including Continuation Application)

For each cooperative agreement year, our funded partners are required to submit an Interim Progress Report, which consists of two separate reports: A 6-month Progress report and a Continuation Application.

For State, Territorial, and Local Agencies and Tribal Governments

Program Announcement 801 – Year 4 6-month Progress Report:
Due October 3, 2011

Program Announcement 801 – Year 5 Continuation Application:
Due November 15, 2011

  • Instructions for locating the application announcement and guidance on grants.gov:
    • Go to www.grants.gov
    • Click on "Apply for Grants" in the red box on the left side
    • Click on "Step 1 - Download a Grant Application Package"
    • Enter "CDC-RFA-DP08-80105CONT12" in the "Funding Opportunity Number" box
    • Click on "Download Package"


For Nongovernmental Organizations

  • Program Announcement DP11-1101 - Year 2 Interim Progress Report
    Due February 17, 2012
     

  • Instructions for locating the application announcement and guidance on grants.gov:

    • Go to www.grants.gov
    • Click on "Apply for Grants" in the red box on the left side
    • Click on "Step 1 - Download a Grant Application Package"
    • Enter "CDC-RFA-DP11-110102CONT12" in the "Funding Opportunity Number" box
