Background

According to the World Health Organization (WHO) and the Joint United Nations Program on HIV/AIDS (UNAIDS) data, Lesotho has the seventh highest tuberculosis (TB) incidence and third highest HIV prevalence rate in the world.  While Lesotho has a relatively small population, the mountainous terrain and dispersed nature of the population makes it difficult to provide access to healthcare.  The Centers for Disease Control and Prevention’s Division of Tuberculosis Elimination (CDC/DTBE) provides ongoing support to CDC/Global AIDS Program (GAP) Lesotho for TB/HIV activities.  This support involves yearly planning of TB/HIV activities, trainings, research and providing technical assistance for TB/HIV.  CDC/DTBE provides technical assistance to the CDC/Global AIDS Program-Lesotho and to the Ministry of Health (MOH) on matters related to diagnosis, treatment, and management of TB, multidrug-resistant TB (MDR TB), and TB/HIV. 

Recent Accomplishments & Ongoing Collaborations

Multidrug-Resistant TB (MDR TB), TB/HIV, and Other At-Risk Populations

National Drug Resistance Survey: In 2008 CDC/DTBE provided technical assistance to the Lesotho National TB Program to design the national drug resistance survey (DRS) and data collection tools, develop data management systems, and support staff collecting data.  DRS establish the burden of disease in a country or region, identifying areas of particularly high or low prevalence which allows governments to target limited resources. The analysis for this DRS was completed in late 2011, and findings and recommendations are being incorporated into national guidelines.

Provide TB/HIV Technical Assistance to the Lesotho In-Country Global AIDS Program Office:  In 2009, CDC/DTBE conducted an assessment of TB/HIV collaborative activities in multiple sites in Lesotho, including HIV testing of TB patients, TB screening of people living with HIV, and appropriate treatment for both.  Recommendations regarding expanded access, lab capacity, screening process, and infection control were given to the MOH and CDC team in-country.
