Background on Healthy People 2020 (HP2020)

Healthy People 2020 outlines the prevention agenda for the nation for the second decade of the 21st century. It is a statement of national health objectives designed to identify the most significant preventable threats to health and to establish national goals to reduce these threats. Healthy People 2020 builds on the previous prevention agenda of Healthy People, Healthy People 2000, and Healthy People 2010.  Healthy People 2020 monitors the progress toward its goals. There are 39 topic areas of Healthy People 2020. An overview of Topic Area 2 (Arthritis, Osteoporosis, and Chronic Back Conditions), is available. Specific arthritis objectives are listed and include 7 of the 8 HP2010 objectives (eliminating racial disparities in total knee replacements disparities did not make the cut), along with 2 new objectives addressing four “specific joint-related activities” and “serious psychological distress.” 

Arthritis-Relevant Healthy People 2020 Objectives

Nine arthritis objectives are included Healthy People 2020, Topic Area 2 (Arthritis, Osteoporosis, and Chronic Back Conditions).

Summary versions of the arthritis objectives are listed below, which we group into two general categories:

Reducing impact

  • AOCBC-1 Reduce the mean level of joint pain.
  • AOCBC-2 Reduce activity limitations.
  • AOCBC-3 Reduce difficulties in performing four specific joint-related activities
  • AOCBC-4 Reduce personal care limitations.
  • AOCBC-5 Reduce serious psychological distress
  • AOCBC-6 Reduce the effect on employment.

Improving interventions

Persons with arthritis are a specifically targeted subgroup for seven additional objectives in the nutrition and physical activity focus areas:

  • AOCBC-7 Increase health care provider counseling for weight and physical activity.
  • AOCBC-8 Increase the proportion of those receiving arthritis education.
  • AOCBC-9 Increase the proportion of those seeing a health care provider for joint symptoms.

Arthritis Relevant Objectives

Persons with arthritis are not a specifically targeted subgroup in other topic areas of HP2020, but there are objectives relevant to arthritis in 8 other topic areas:

Disability and Health

  • DH-9: (Developmental) Reduce the proportion of people with disability who encounter barriers to participating in home, school, work, or community activities.
  • DH-10:  (Developmental) Reduce the proportion of people with disabilities who report barriers to obtaining the assistive devices, service animals, technology services, and accessible technologies that they need.
  • DH-13:  (Developmental) Increase the proportion of people with disabilities who participate in social, spiritual, recreational, community, and civic activities to the degree that they wish.
  • DH-15 & 16:  employment
  • DH-18:  (Developmental) serious psychological distress.

Educational and Community-based Programs

  • ECBP-8:  (Developmental) Increase the proportion of worksites that offer an employee health promotion program to their employees.
  • ECBP-9:  (Developmental) Increase the proportion of employees who participate in employer-sponsored health promotion activities.

Health-Related Quality of Life & Well-Being

Mental Health and Mental Disorders

Nutrition and Weight Status

  • NWS-6:  Increase the proportion of physician office visits that include counseling or education related to nutrition or weight.
    • NWS-6.2:  Increase the proportion of physician office visits made by adult patients who are obese that include counseling or education related to weight reduction, nutrition, or physical activity.
  • NWS-8:  Increase the proportion of adults who are at a healthy weight.
  • NWS-9:  Reduce the proportion of adults who are obese.

Older Adults

  • OA-3:  (Developmental) Increase the proportion of older adults with one or more chronic health conditions who report confidence in managing their conditions.
  • OA-5:  Reduce the proportion of older adults who have moderate to severe functional limitations.
  • OA-6:  Increase the proportion of older adults with reduced physical or cognitive function who engage in light, moderate, or vigorous leisure-time physical activities.

Physical Activity

  • PA-1:  Reduce the proportion of adults who engage in no leisure-time physical activity.
  • PA-2:  Increase the proportion of adults who meet current Federal physical activity guidelines for aerobic physical activity and for muscle-strengthening activity.
  • PA-11:  Increase the proportion of physician office visits that include counseling or education related to physical activity.
  • PA-12:  (Developmental) Increase the proportion of employed adults who have access to and participate in employer-based exercise facilities and exercise programs.
  • PA-13: (Developmental) Increase the proportion of trips made by walking.
  • PA-14: (Developmental) Increase the proportion of trips made by bicycling.

Sleep Health

  • SH-4:  Increase the proportion of adults who get sufficient sleep.

Arthritis-Relevant Healthy People 2010 Objectives (Historical)

Eight first ever arthritis objectives were proposed in the 2000 version of Healthy People 2010, Focus Area 2, about arthritis, osteoporosis, and chronic back conditions. There has also been a Healthy People 2010 Midcourse Review of all Focus Areas, formal 2006 Progress Review for Focus Area 2. These deliberations resulted in eight modified arthritis objectives (including two with sub-objectives) in Healthy People 2010. The modifications, their rationale, and the targets for each arthritis objective can be found in the Focus Area 2 chapter of the Midcourse Review, along with the full and final language of the objective.   • 22-3 Increase the proportion of adults who engage in vigorous physical activity.
  • 22-4 Increase muscular strength and endurance.
  • 22-5 Increase flexibility
