If the clinic name has changed since 2008, the current name is listed in italics directly under the 2008 name. If the clinic location has changed since 2008, the clinic is listed alphabetically by the current city and state.

Clinic names preceded by the § symbol have reorganized or closed since 2008. Reorganization is defined as a change in ownership or affiliation or a change in at least two of the three key staff positions (practice director, medical director, or laboratory director).
