Web Claim Form

Use this Web form to claim Federal depository materials from GPO-issued shipping lists. These lists will have GPO as the claim address. Most microfiche claims must be made through the appropriate microfiche contractor and claims for USGS maps must be sent to USGS. Claim only documents in your selection profile that you did not receive.

Claims can be submitted from one shipping list at a time. You can submit, reset and reuse this form as many times as necessary. Enter the shipping list number, item, and class number for each item being claimed. If all the documents on a shipping list need to be claimed, check the "Claim Entire Shipping List" box below.

NOTE: Please first check the Claims Copies Exhausted page before making a claim. This page is updated periodically as necessary, and lists those publications for which GPO has exhausted its claims stock. Please do NOT submit a claim for any publication appearing on this list.

Expired Shipping Lists: Claims may not be made against the lists because the 60 day claiming period has expired.
