Determine Your Eligibility

Before receiving services with the VA Salt Lake City Health Care System, you should first determine your eligibility. Visit our enrollment and eligibility page for more information.

To Make, Change, or Cancel an Appointment

Eligible veterans should contact the VA Salt Lake City Health Care System to schedule an appointment with a primary care provider.  If you have an existing appointment and cannot keep it, let us help you change it.  Here is more information to help you make, change, or cancel your appointment.

Advance Directives

There may come a time when you are unable to decide or tell us what you want.  You have the opportunity now to make some decisions ahead of time.  Tell us in writing about the kind of care you want if you cannot speak for yourself.

Patient Education

Patient education is an important part of your health care during clinic visits, hospitalization and following discharge from the hospital. We want to assist you in learning more about your condition, how to living a healthy lifestyle, and ways to self-manage your own health.  A number of educational resources are available to you at no cost.

Customer Service Center

We want to help you understand and exercise your rights as a patient. Your comments and recommendations for changes are always welcome and help us improve how we provide health care.

You can contact Trent Thompson, Acting Patient Liaison Officer at (801) 582-1565 (or 1-800-613-4012), ext. 1900. The Patient Liaison office is located at the George E. Wahlen Department of Veterans Affairs Medical Center, Building 1, First Floor, Room 1B24.

Patient Services Center

The Patient Services Center assists the patients, their families, and the staff coordinating the arrangements for admission, transfers, lodging and outpatient care for long distance patients.  The Patient Services Center is located on the fourth floor of building 1. Office hours are Monday through Friday, 8:00 a.m. to 4:30 p.m. Contact them at (801) 584-5626 or toll free 1-800-613-4012, ext. 5626.

Billing Inquiries

To speak with someone about billing inquiries, call the Medical Center at (801) 582-1565 (or 1-800-613-4012), then press 7.

Release of Information

The Release of Information Office processes requests for medical records and assists providers with completing forms for patients.  Let our staff help you get the information you need.
