GENERAL

How do I set up an account for my unit?

You need to submit a DA Form 12-R. The directive governing the DA Form 12-R is PAM 25-33. Both can be downloaded or browsed from our web site. Go through your installation Directorate of Information Management (DOIM). You can then FAX it to DSN 693-9620 or Comm (314) 592-0923, ATTN: Account Processing. My unit is moving, how do we change the address?

The new information needs to be submitted on a DA Form 12-R. You can fax it to DSN 693-9620 or Comm (314) 592-0920, Attn: Account Processing. To ask any questions call a member of the Account Processing Team at DSN 693-9606 or Comm (314) 592-0910. The directive governing the DA Form 12-R is PAM 25-33. Both can be downloaded or browsed from our web site.

How do I close my account?

You need to submit a DA Form 12-R, (complete blocks 1-6, 9 and 13 or send a memo containing the same information (including Commander’s signature) on your organization’s letterhead. You can then FAX it to DSN 693-9620 or Comm (314) 592-0920, ATTN: Close Account. Any questions, call a member of the Account Processing Team at DSN 693-9606 or Comm (314) 592-0910. As with any other changes to your account status, follow additional local procedures, if applicable. Contact your installation Publications Control Officer or Directorate of Information Management (DOIM) for local guidance.

Do I still need to forward a DA Form 12-R each year to validate my account?

If you have validated your address with the Point and Click Ordering System, you no longer have to submit a DA Form 12-R. If you have changes to your address, the annual validation requirement still exists.

How do I obtain a copy of my DD Form 214 (Certificate of Release or Discharge from Active Duty) as well as other documents in my official file?

Veterans may request copies of their DD Form 214 and/or copies of documents from their official records from the National Archives and Records Administration (NARA). If deceased, his/her next of kin (parents, spouse, or children) may request copies. Online, you may go to either www.archives.gov/veterans and complete the eVetRecs or SF 180, or go to: www.hrc.army.mil/site/Reserve, go to Other Links and select Veterans and Retirees. In addition, SF 180 may be available from your local Veteran's Administration office.

Alternately, requests may be made by submitting a letter to the National Personnel Record Center, ATTN: Military Personnel Records, 9700 Page Boulevard, St. Louis, MO 63132-5100. Include as much information as possible: your full name, address, service number, SSN, branch of service, dates of service, and date and place of birth. This letter must be signed and dated. Customer service at this location is at 314-801-0800.

How do I install the DA PAM 25-30 (Consolidated Index of Army Publications and Blank Forms)?

We currently offer the DA Pam 25-30 in two ways. —

–First as an online web application database, see our homepage website and click on the link for PAM 25-30 (Product Catalog) located on left-side navigation .

–Second we have replaced the CD-ROM (EM 0001) version of the DA PAM 25-30 with an application that is downloaded and installed to your workstation. See the link (Offline version) on the PAM 25-30 webpage described above. Note You may require administrative rights to install this application on your workstation. Please check with your local IT staff prior to installing this or any software application.

How do I obtain printed and stocked Departmental Forms (DA, DD, SF, OF and other agency forms) used by the Army?

If you are an Army user and have an account with MDD you may order online at: https://dol.hqda.pentagon.mil/ptclick. ELECTRONIC PUBLICATIONS

What are PDF files?

Adobe has created a type of file that looks the same on any computer when opened with Adobe Acrobat Reader. These files allow for special tables, graphics, and typefaces that cannot be created any other way for the World Wide Web. Adobe named this type of file Portable Document Format or PDF.

Do I need special software to read PDF files ?

Yes, you'll need to download (free) the Adobe Acrobat Reader to read these files. You can follow this link to the Adobe download site, which includes instructions about installing the Reader software.

Unable to open a PDF file, all I get is a blank page.

There is a 6.0 Adobe problem that requires a fix [12/18/2003].

According to Adobe, the Acrobat 6.0.1 and Adobe Reader 6.0.1 updates fix a number of problems discovered after Acrobat 6.0 and Adobe Reader 6.0 were released. One problem of note is the PDF files could not be opened from secured (HTTPS) Web sites that use Netegrity site management software (such as AKO). Please review the Adobe Downloads page to assure you have the latest files Adobe Downloads.

I can't find the publication or form listed on this web site. How can I check to see if it is still available or current?

You should always check with your local publications control officer, DOIM or check the DA PAM 25-30 to determine current availability. Please note; NOT every document is available in electronic format. Record details found when searching the DA PAM 25-30 will indicate availability of electronic files and the web site the file can be located on if available.

FORMS

What is the Forms Content Management Program (FCMP)?

The FCMP is the new official Army standard for Forms, digital signature, and electronic information processing. The FCMP integrated enterprise solution consists of Lotus forms and digital signature (ApproveIt) software purchased for the Army enterprise 1.4 million army wide users. The FCMP provides an enterprise solution for providing not only fillable forms and storage of data, but it will also enable a form to be pushed from pc to pc using digital signature and workflow techniques. This program provides the U.S. Army the means to develop one single enterprise solution in automating functional business processes army wide, eliminating duplicative stovepipe systems being developed across the army. In addition, with the ApproveIt digital signature capability in place this will eliminate army resources for being expended on courier, transportation, mailing (FedEx), handling, cost, etc.

What will replace the FormFlow solution for electronic e-forms design and usage?

The PureEdge Suite of forms software has been selected to replace FormFlow as the new standard for all Army electronic forms. These PureEdge products, along with ApproveIt Desktop electronic approval software from Silanis, will be required for designing and using Army forms.

What is the PureEdge e-form?

PureEdge e-forms represent a quantum leap over previous forms software and are far more than simply another electronic forms solution. Under the new program, the printed form may be one of many potential outputs of what will comprise the new system. The PureEdge e-form is an intelligent form that encapsulates the form logic and the underlying data into a single file in a non-proprietary Extensible Markup Language (XML) format. Secure electronic signatures, automatic routing of the form to recipients, and support for non-connected users are just some of the benefits of the e-form. Additional features include automatic information population, synchronization of form data with Army databases, and integration with functional business processes.

What software do I need to use/read Pure Edge e-forms?

You must have Pure Edge software installed on your personal computer to use electronic fillable forms. In order to digitally sign a signature enabled electronic form you will need to install Silanis ApproveIT software. NOTE: At this time, not all forms have been signature enabled.

How can I get the PureEdge Viewer and ApproveIt software?

The Army-tailored PureEdge Viewer v6.5 software, along with the Army-tailored Silanis ApproveIt Desktop v5.7.1 electronic approval software is available through the Army Golden Master IV or via download from the Army Small Computer Program website.

After the conversion of all Departmental forms to PureEdge, will I still be able to access the FormFlow version from the APD Web site?

Yes, once all of the Departmental forms in FormFlow have been converted into the PureEdge format, the FormFlow versions will remain in APD's master catalog for at least a year to provide a transition period to the field.

Pure Edge versions of the Army library of forms is available on the Army Publishing Directorate's web site and the Army Knowledge Online (AKO) My Forms Portal.

Are electronic forms available in PDF format?

Yes. APD has converted its forms library to Portable Document Format (PDF) as an additional way for you to access our electronic forms. Our PDF forms are not fillable. However, they can be printed to be filled out manually. We will continue to host only PDF forms required by the general public.

If the addition is a new publication or it's in the process of being revised, it will be sent to you automatically. If the publication is in stock at the warehouse, you will have to requisition it.

You can do a Resupply Activity Report query from our site. It will show you the last 180 days worth of orders and their statuses.
