Headquarters, Department of the Army (HQDA) Telework policy and application are now available on the web.

New, Improved Ordering System

Coming soon -- an improved Ordering System, which will provide you enhanced capabilities. You will be able to:

- Scroll through the search results when ordering, instead of using page forward/back.
- Delete requirements directly from the Subscription Report.
- Copy reports to an Excel spreadsheet to enable you to better manage your publications/forms requirements.

Access will be available from the current (APD) web site, which will automatically link you to the new system on the Directorate of Logistics-Washington web site.

Secretary of the Army Publishing Awards -- Nominations due to APD NLT 15 November 2012

The nomination due date for the 2012 Secretary of the Army Awards (Publications Improvements) has been extended to 15 November 2012. The 2012 awards recognize contributions made between 1 July 2011 and 30 June 2012.
AR 25-30, Chapter 6, provides detailed information about these awards, to include specific categories of nominations. Each nomination package should include one hard copy and three CDs of the entire award submission package.

DD Form 1805 (US District Court Violation Notice)

The DD Form 1805, US District Court Violation Notice, will no longer be managed, stored or distributed from the Media Distribution Division (MDD) in St. Louis, MO. Effective immediately, customers can now receive the form free of charge from the Central Violations Bureau (CVB). To request the forms, visit the following website: http://www.cvb.uscourts.gov/vn Enter the login name: AGENCY Enter the password: TICKETS. Click the button to "Request Violation Notices". To submit a request, complete the form and click the button at the bottom of the page. A valid CVB location code is required to order violation notices. All questions regarding the DD Form 1805 can be directed to the CVB at 1-800-827-2982 Monday through Friday between the hours of 8:00 am to 5:00 pm Central Time (CT).

Transition Period On Revised OER And NCOER Support Forms

Officer Evaluation Report and Noncommissioned Officer Evaluation Report forms and support forms have been revised. These include DA Forms 67-9, 67-9-1, 67-9-1A, 2166-8, 2166-8-1. The revised forms are dated MAR 2006. The Academic Evaluation Report forms (DA Forms 1059 and 1059-1) have also been revised, and are dated APR 2006. The previous edition of these forms will remain available on the APD websites and MY FORMS Portal until 12 OCT 2006, as part of the Military Evaluations Policy and Forms Transition Plan. The transition allows for a 4-month period in which both the revised forms and previous edition forms may be used.
