Billing Authorization form (BAF)

These documents are for use by CTIVS and its customers.

Billing Authorization Form (BAF): This form is used to document an agreement between CTIVS and our customer which indicates the services or goods to be provided by CTIVS, and the maximum amount that the customer has agreed to pay for those items. (document currently under review)

CIT Account Codes

A CIT Account Code is the accepted billing mechanism necessary to request a CIT system or service. A CIT Account Code is linked to a more general CAN (Common Account Number) which is specific to a particular type of service. For example, your organization might have one CIT Account Code which is used for teleconferencing services, and another CIT Account Code which is used for equipment maintenance. Although CANs are still in use by various organizations, they are no longer accepted for CIT billing agreements.

CIT Account Code Request Form (NIH): If you work for an NIH Institute or Center, please use this version to request that a new CIT Account Code be generated for your project. (PDF, < 1 MB)

CIT Account Code / Interagency Agreement Request Form (non-NIH): If you work for an HHS Operational Division, other than an NIH Institute or Center, please use this version to request that a new CIT Account Code be generated for your project. (PDF, < 1 MB)

Annual Renewal of Interagency Agreement: If you already have an Interagency agreement in place, please use this form to continue using the same agreement. (PDF, < 1 MB)

Deregistration Official Authorization for CIT Accounts: This form is used to establish or change the designation of Deregistration Officials within an IC or organization. (PDF, < 1 MB)
