Opioid Treatment Program Mortality Report FormThank you for submitting mortality reports to SAMHSA. The data submitted to date has allowed SAMHSA to monitor trends in opioid mortality. At this time, we are temporarily ceasing the collection of data from opioid treatment programs. We will notify you shortly of our plans to continue with the collection process.
