This series was developed by the VHA National Center for Health Promotion and Disease Prevention and the Agency for Healthcare Research and Quality (AHRQ) and covers selected recommendations from the US Preventive Services Task Force (USPSTF).  Providers and patients can use this series to enhance communication and make more informed decisions about preventive health care.

• Each clinician fact sheet and patient brochure has a high resolution PDF file for downloading and printing.
• The patient brochures can be customized on the back center panel with local facility information prior to printing.
• Print copies of the patient brochures can also be ordered from the VA Forms Depot through the facility Forms Officer.
