Other Identification Numbers Page
Provide us with any provider identification numbers
currently in use or formerly used by you. (Optional) The numbers
may include, but are not limited to:
Medicare PIN, Medicare UPIN, Medicare OSCAR/Cerfitication, Medicare NSC and
Medicaid State numbers.
These numbers may be of use in matching your NPI record to
health plans' records.
If you don't have such numbers, you are not required to obtain
them for purposes of applying for an NPI.
If you are in doubt about whether to include them, please contact
the health plans with which you do business. You may add numbers
later if you find a need.
Numbers added previously as UPIN and Medicare will now be
displayed as Medicare UPIN and Medicare PIN to match the CMS-10114 form.

If you enter a SSN/ITIN/EIN in the Number field you will be shown a warning message that
an invalid entry has been made and to update this field. If you chose to Save the invalid
entry NPPES will convert it into a character string to protect the SSN/ITIN/EIN
from being disseminated to downstream systems in clear text. If you entered a SSN it will be converted into 9
dollar signs ($), an EIN will be converted into 9 equal signs (=) and an ITIN will be converted
into 9 asterisk (*). Only numbers that exactly match the SSN/ITIN/EIN entered on your
record will be converted, if your SSN/ITIN/EIN report in NPPES changes the previously
converted numbers will now display.
