The student's first name

You must enter the student's first name.

Use the student's proper name, not a nickname. The student's first name must match the first name on his/her Social Security card.

The student's entry must contain only letters (A-Z), numbers (0-9), periods (.), apostrophes ('), dashes (-), or blanks (spaces). No other characters are allowed.

The student's full last name

You must enter the student's last name.

Use the student's proper name, not a nickname. The student's last name must match the last name on his/her Social Security card.

The student's Social Security Number

For an extra level of security, select the Virtual Keyboard icon and use your mouse to select the characters.

You must enter the student's Social Security Number.

Enter the Social Security Number (SSN) that is printed on the student's Social Security card. Enter this number without dashes. For example, enter 123456789.

The student's date of birth

For an extra level of security, select the Virtual Keyboard icon and use your mouse to select the characters.

You must enter the student's date of birth.

Enter the date of birth in "mmddyyyy" format, and make sure it exactly matches the date of birth on file with the Social Security Administration. For example, if the student's date of birth is May 3, 1980, type 05031980.
