To use the DQS, you must first read and accept the data use agreement below.

Data Use Agreement

The DQS does not query databases that contain personal identification information on survey participants. The information you access through this system may not be used for any purpose other than for legitimate research, decision making, statistical reporting, and analysis. Any effort to determine the identity of any participants is prohibited by law under sections 308(d) and 903(c) of the Public Health Service Act (42 U.S.C. 242m and 42 U.S.C. 299 a-1). If the identity of any person or establishment should be discovered inadvertently, then no use should be made of this knowledge, the DRC should be notified of this discovery, the information identifying any individual will be safeguarded or destroyed, and no one else will be informed of the discovered identity.

Therefore, as a condition to my access to data provided by the DQS, I agree to the following:

1. Use the data for statistical reporting and analysis only.
2. Make no attempt to learn the identity of any individual included in these data.
3. Make no disclosure or other use of the identity of any person discovered inadvertently and advise the DRC of any such discovery.

I understand that failure to adhere to the above agreement will result in loss of access to the DQS, and I may be subject to legal penalties. Any use, release, or publication of health data contrary to the provisions stated may be punishable by law.
