The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (hubzone, 8(a), small, small disadvantage, service disabled veteran owned small business, or large business) relative to NACIS 624190.



Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on FBO. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement.



The Department of Veterans Affair, Network Contracting Office 22, is seeking sources for a potential contract to provide case management and representative payee services for VA Greater Los Angeles Healthcare System (VAGLAHS)



Services may include, but are not limited to the following:



1: The Contractor will be station on site to provide case management and payee services to selected Veterans in the Dual Diagnosis Program. Cost shall be inclusive of all necessary staff and supervision in order to provide the services required within the contract. There shall be no additional cost to the Government or to the Veterans.



2: It is necessary to have an establish working relationship with Social Security Administration (SSA) in order to continue services, at no time will there be a lapse in service due to the financial and health risk of the Veterans.



3: VA case managers, participants and the contractor will develop a personal budget for each participant. This budget shall be crafted to ensure that all basic necessities are met, such as; food shelter, and medical care.



4: Financial Reporting: Contractor shall keep record of each participant and file monthly report of all income and expenditures. Records maybe audited at any time by VHA, VBA, or SSA. Vendor is required to account for the funds received by completing a Representative Payee Report, Form SSA-623. And file all required annual documentation to VHA, VBA, IRS and SSA.





The intended contract period will be for a base year and four (4) one year option period, which may begin June 1, 2013. The service will be performed at:
