The website works best in Internet Explorer versions 6.0 and higher and Firefox versions 2.0 and higher. Users may experience issues with other browsers and are recommended to use the browsers listed above. It is recommended that browser windows be opened using the icon on the desktop to avoid shared browser sessions. Some browsers share sessions regardless of how the browser is opened. Please check with the browser's vendor about session management. When NPPES detects multiple browsers open within the same session, NPPES will terminate the session to protect the data in NPPES. Data entered will be lost and will need to be re-entered. Use of the Back and Forward browser buttons could result in loss of all the information entered. Users should use the Next and Previous buttons provided on the application to navigate between the pages of the application.

You must provide an address where we can contact you directly to resolve any issues that may arise during our review of your application. (Required)  This information will assist us in contacting you with any questions we may have regarding your application for an NPI or with other information regarding NPI. The Address Line 1, Address Line 2, and City fields allow the following special characters: ampersand, apostrophe, colon, comma, forward slash, hyphen, left and right parentheses, period, pound sign, quotation mark, and semi-colon. In addition, the Address Line 1 and Address Line 2 fields allow the "at" sign. A field cannot contain all special characters.

The Phone number, Extension, and Fax fields on the Mailing Address page only accept digits.

If you select a country (other than the United States) that does not list a province, enter the country’s name in the province field.

Military Addresses can only be entered on the Military Mailing Address Page. The City should be APO or FPO, the State should be AA, AE or AP, and the Country should be United States.

For example, the address USS NIMITZ CVN 68 FPO AP 96620-2820 would be entered as:
Address Line 1: USS NIMITZ
Address Line 2: CVN 68
City: FPO
State: AP
Zip+4: 96620-2820

For example, the address Ramstein AFB 435 MSS/DPF, Unit 3220, Box 410, APO, AE, 09094 would be entered as:
Address Line 1: Ramstein AFB 435 MSS/DPF

City: APO
State: AE
Zip+4: 09094

Military Address State options:
      AE (Armed Forces Europe / Canada / Middle East / Africa)
      AP (Armed Forces Pacific)
      AA (Armed Forces Americas)

Military Address City options:
      APO - Army Post Office / Air (Force) Post Office
      FPO - Fleet Post Office

NOTE: Do not report your residential address unless it is also your Business Mailing Address

NOTE: SSN/ITIN/EINs can only be entered in the SSN/ITIN/EIN field. If an SSN/ITIN/EIN is entered in any other field, NPPES will display an error message, indicating that an SSN/ITIN/EIN has been entered in a non SSN/ITIN/EIN field. You are required to update those fields in order to continue to the next page in the application. If a 9-digit contiguous number is entered in a non numeric field and the 9-digit number is not the SSN/ITIN/EIN entered on the record, NPPES will display a warning message, indicating that a 9-digit contiguous number has been found in a non numeric field when the Next button is clicked. You can either update the field value or click the Next button again to accept the 9-digit number.
