The website works best in Internet Explorer versions 6.0 and higher and Firefox versions 2.0 and higher. Users may experience issues with other browsers and are recommended to use the browsers listed above. It is recommended that browser windows be opened using the icon on the desktop to avoid shared browser sessions. Some browsers share sessions regardless of how the browser is opened. Please check with the browser's vendor about session management. When NPPES detects multiple browsers open within the same session, NPPES will terminate the session to protect the data in NPPES. Data entered will be lost and will need to be re-entered. Use of the Back and Forward browser buttons could result in loss of all the information entered. Users should use the Next and Previous buttons provided on the application to navigate between the pages of the application.

Contact Person Information Page
The contact person is the person who we will contact if we have any questions regarding your NPI application or change request. This is the person who will be notified of your NPI assignment via e-mail at the e-mail address provided on this page.
1. Select the "Same As..." button if you want your contact person to be the provider (if an individual) or the authorized official (if an organization) named on the Provider Profile Page. You must still enter the phone number and e-mail address for this person. For Individual providers, the “Same As Mailing Phone” or “Same As Practice Phone” buttons can be selected to copy the corresponding data into the Contact Person Phone Number and Extension fields. You must still enter the e-mail address for this person.

2. If you do not select the "Same As..." button, you must provide alternate contact person information. Enter the full name, credentials, and title of the contact person as well as the phone number and e-mail address for this person. The First, Middle, Last, Credential(s), and Title fields allow the following special characters: ampersand, apostrophe, colon, comma, forward slash, hyphen, left and right parentheses, period, pound sign, quotation mark, and semi-colon. A field cannot contain all special characters. The e-mail address fields allow the following special characters: "at" sign, hyphen, period, and underscore.

3. The Phone number and Extension fields only accept digits.

NOTE: SSN/ITIN/EINs can only be entered in the SSN/ITIN/EIN field. If a SSN/ITIN/EIN is entered in any other field, NPPES will display an error message, indicating that a SSN has been entered in a non SSN field. You are required to update those fields in order to continue to the next page in the application. If a 9-digit contiguous number is entered in a non numeric field and the 9-digit number is not the SSN/ITIN/EIN entered on the record, NPPES will display a warning message, indicating that a 9-digit contiguous number has been found in a non numeric field when the Next button is clicked. You can either update the field value or click the Next button again to accept the 9-digit number.
