The website works best in Internet Explorer versions 6.0 and higher and Firefox versions 2.0 and higher. Users may experience issues with other browsers and are recommended to use the browsers listed above. It is recommended that browser windows be opened using the icon on the desktop to avoid shared browser sessions. Some browsers share sessions regardless of how the browser is opened. Please check with the browser's vendor about session management. When NPPES detects multiple browsers open within the same session, NPPES will terminate the session to protect the data in NPPES. Data entered will be lost and will need to be re-entered. Use of the Back and Forward browser buttons could result in loss of all the information entered. Users should use the Next and Previous buttons provided on the application to navigate between the pages of the application.

Provide information on your other identification numbers (Optional). If you select a number type of "Medicaid", you will need to enter the issuing state. If you select a number type of "Other", you will need to enter a description of the entity that assigned that identifier to you. If you don't have such numbers, you are not required to obtain them for purposes of applying for an NPI.

1. The Number and Issuer fields allow the following special characters: ampersand, apostrophe, colon, comma, forward slash, hyphen, left and right parentheses, period, pound sign, quotation mark, and semi-colon. In addition, the Description field allows the "at" sign. A field cannot contain all special characters.

Description of other identification numbers:
Medicaid – An identifier assigned to a provider by a State Medicaid program. If a Medicaid identifier is reported, the name of the State must be reported.

Medicare NSC – An identifier assigned to a durable medical equipment, prosthetics, orthotics, and supplies (DMEPOS) supplier by the National Supplier Clearinghouse (NSC).

Medicare OSCAR/Certification – An identifier assigned to an institutional provider of services (a certified provider) by CMS regional office/State survey-certification staff.

Medicare PIN – An identifier assigned to a Part B provider/supplier by the carrier or A/B MAC. Medicare PIN also includes PINs that are assigned to Medicare Part B providers by the Railroad Retirement   Board.

Medicare UPIN – An identifier that had been assigned to physicians and certain other practitioners by the UPIN Registry. UPINs are no longer assigned.

Other, Specify – An identifier assigned by a health   plan other than Medicare or Medicaid (e.g., Blue Cross, Blue Shield, Aetna, Kaiser-Permanente, etc.). If an identifier is reported in Other, Specify, the name of the health plan that assigned the identifier   must be reported as the Issuer.

2. Click the Save button, to enter the identifier into the system. After clicking the save button you will be returned to the Other Identification Numbers Page, where the identification number you added will be displayed in a table.

3. Click the Save and Add Another button to save the entered identifier and to add an additional identifier.

4. Click the Cancel button to return to the Other Identification Number Page without saving the identifier.

5. Providers who enter a SSN/ITIN/EIN in the License Number field will be shown a warning message that an invalid entry has been made and to update this field. Providers who choose to Save the invalid entry will have the SSN/ITIN/EIN converted into a character string to protect the SSN/ITIN/EIN from being disseminated to downstream systems in clear text. The SSN will be converted into 9 dollar signs ($), the EIN will be converted into 9 equal signs (=) and the ITIN will be converted into 9 asterisk (*). Only numbers that exactly match the SSN/ITIN/EIN entered on the provider’s record will be converted, if the provider’s SSN/ITIN/EIN changes on the record, the previously converted numbers will now display.

NOTE: DO NOT report the Social Security Number (SSN), IRS Individual Taxpayer Identification Number (ITIN) in the Number field.
