Application Form

To receive tissue microarrays from the NCI Cancer Diagnosis Program (CDP) you must submit an application that uses the following outline and includes all of the requested information. Incomplete applications will be returned for completion resulting in a delay in the evaluation of your request.

  1. Principal Investigator Information:
    Please provide Name, Institution, Department, Street Address, Country, Phone, Fax and E-mail.
  2. How Did You Learn About CDP Tissue Microarrays:
    Colleague, Professional Meeting (Specify), NCI/NIH, Internet, Advertisement (Specify), Other (Specify)
  3. Type of TMA Requested and Number of Sets Requested:
    Please consult the detailed information about the design of the TMAs on this Web site at http://cdp.nci.nih.gov/tma_types.html.
  4. Scientific Rationale:
    1. Concise introduction to the project concluding with a brief statement of the hypothesis of the proposed research.
    2. Specific aims of the work to be carried out with the TMAs.
    3. Brief description of each marker to be evaluated.
  5. Scientific Approach:
    1. Please provide a brief description of the technical approach (measurement techniques and reagents).
    2. Please describe your experience using this approach with human paraffin-embedded sections in your laboratory.
    3. Please describe the scoring system you plan to use.
    4. What do you plan to use for positive and negative controls?
    5. Please describe the statistical method(s) you will use to test the hypothesis and analyze the data.
