New Affordable Care Act Demonstration to provide care at home for Medicare patients
Up to 10,000 Medicare patients with chronic conditions will now be able to get most of the care they need at home under a new demonstration announced today by the Centers for Medicare & Medicaid Services (CMS).

Partnership for Patients Hospital Engagement Networks
Hospitals across the country will have new resources and support to make health care safer and less costly by targeting and reducing the millions of preventable injuries and complications from healthcare-acquired conditions. As a part of the Partnership for Patients initiative, Hospital Engagement Networks will help identify solutions already working to reduce healthcare-acquired conditions and preventable hospital readmissions, and work to spread them to other hospitals and health care providers.

Health Care Innovation Awards
Up to $1 billion dollars will be awarded to innovative projects across the country that test creative ways to deliver high-quality medical care and save money. Launched today by the Department of Health and Human Services, the Health Care Innovation Awards will also give preference to projects that rapidly hire, train, and deploy health-care workers.

Federally Qualified Health Center Advanced Primary Care Practice Demonstration
500 community health centers in 44 States across the country will receive approximately $42 million over three years to improve the coordination and quality of care they deliver to people with Medicare and other patients. Under this demonstration, Medicare will pay community health centers based on the quality of care they deliver. This improved payment system will reward clinics for such things as helping patients manage chronic conditions like diabetes or high blood pressure.

HHS announces new incentives for providers to work together through Accountable Care Organizations when caring for people with Medicare
People with Medicare will be able to benefit from a new program designed to encourage primary care doctors, specialists, hospitals, and other health care providers to coordinate their care under a final regulation issued today by the Department of Health and Human Services (HHS). Created by the Affordable Care Act, these final rules on Accountable Care Organizations add to the menu of options for providers looking to better coordinate care for patients and will make it easier for providers to deliver high-quality care and use health-care dollars more wisely.

Innovation Advisors Program
The Centers for Medicare and Medicaid Services is now accepting applications for a new Innovation Advisors program to help health professionals deepen skills that will drive improvements to patient care and reduce costs. These health-care improvements will benefit people enrolled in Medicare, Medicaid, and the Children's Health Insurance Program (CHIP). Made possible by the Affordable Care Act, this initiative will be managed by the Center for Medicare and Medicaid Innovation (Innovation Center).

HHS launches new Affordable Care Act initiative to strengthen primary care
The U.S. Department of Health and Human Services (HHS) today launched a new initiative made possible by the Affordable Care Act to help primary care practices deliver higher-quality, more-coordinated, and patient-centered care. Bundled Payment for Care Improvement initiative
The Bundled Payments for Care Improvement initiative seeks to improve patient care through payment innovation for health-care services that foster improving coordination and quality through a patient-centered approach. The CMS Innovation Center is seeking applications for four broadly-defined models of care, three of which would involve a retrospective bundled payment arrangement, with a target price (target payment amount) for a defined episode of care.

Physician Group Practice Demonstration succeeds in improving quality and reducing costs
Today, the Centers for Medicare and Medicaid Services (CMS) announced results from the initial Physician Group Practice (PGP) Demonstration, a landmark partnership with physician group practices that aims to better coordinate care across different settings, leading to improved quality and cost savings. After five years, this demonstration has shown positive results, including significant progress in areas of both quality improvement and savings in Medicare expenditures.

Partnership for Patients Meets Goal of Over 2,000 Hospitals Participating
U.S. Department of Health and Human Services (HHS) Secretary Kathleen Sebelius announced today that nearly 4,500 organizations — including over 2,000 hospitals — have pledged their support for the Partnership for Patients, the Obama Administration’s new nationwide patient safety initiative. In less than three months, the Administration has met its goal of having 2,000 hospitals pledge their support.
