Workers' Compensation Forms

You must report any workplace related injury as soon as possible to the Division of Occupational Health and Safety (DOHS), Occupational Medical Service (OMS). OMS will furnish all necessary forms and direct you to the NIH Workers' Compensation Programs. If you have misplaced your form, you may request another copy from OMS or attach the appropriate instructions to the downloadable form from the US Department of Labor (DOL) below. If you are downloading a CA-1 or CA-2, insert the following in item 17 before sending the form to your supervisor:
