Part B - Grants to States & Territories

Part B of the Ryan White HIV/AIDS Program provides grants to States and U.S. Territories. Part B grants include a base grant, the AIDS Drug Assistance Program (ADAP) award, ADAP supplemental grants, grants to States for Emerging Communities, and an award for Minority AIDS Initiative activities. Grantees are State departments of health or other State entities responsible for implementing and managing state public health programs.

Eligibility

All 50 States, the District of Columbia, Puerto Rico, Guam, the U.S. Virgin Islands, and the five U.S. Pacific Territories or Associated Jurisdictions are eligible for funding. Emerging Communities are defined as those reporting between 500 and 999 cumulative reported AIDS cases over the most recent 5 years.

Funding Considerations

All Part B funding is distributed through formula and other criteria.
Base Part B grants are awarded to States and Territories using a formula based on reported living cases of HIV/AIDS. States with more than 1 percent of total HIV/AIDS cases reported in the United States during the previous 2 years must provide matching funds with their own resources using a formula outlined in the legislation.
Additional Part B funds are "earmarked" for State ADAPs, which primarily provide medications. Fundable services include treatment adherence and support as well as health insurance coverage with prescription drug benefits. Five percent of the ADAP earmark is reserved for grants to States and Territories that have a severe need for medication assistance.
Part B provides a formula grant of $8.4 million for the Minority AIDS Initiative to provide education and outreach services to increase the number of eligible racial and ethnic minorities who have access to treatment through the Part B ADAPs.
A competitive supplemental grant program, authorized in 2006, based on demonstrated need criteria is now available. (This program has been funded from increases in the Part B base and unobligated balances.)

Services and Implementation

Part B grants fund core medical services and support services. Core medical services include outpatient and ambulatory health services, ADAP, AIDS pharmaceutical assistance, oral health care, early intervention services, health insurance premium and cost-sharing assistance, home health care, medical nutrition therapy, hospice care, home and community-based health services, mental health, outpatient substance abuse care, medical case management, and treatment adherence services.

Support services that are needed for people with HIV/AIDS to achieve their medical outcomes include respite care for caregivers of people with HIV/AIDS, outreach services, medical transportation, linguistic services, and referrals for health care and support services.

Grantees are required to spend at least 75 percent of the award on core medical services and no more than 25 percent on support services.

Part B providers may include public or nonprofit entities. For-profit entities are eligible only if they are the sole available providers of quality HIV care in the area.

Most States provide some services directly, while others work through subcontracts with Part B HIV Care Consortia. A consortium is an association of public and nonprofit health care and support service providers and community-based organizations that plans, develops, and delivers services for people living with HIV disease. Services provided through a consortium are considered support services.

Quick Facts

For FY 2010, $1.25 billion was appropriated for Part B programs, and $835 million was earmarked for ADAP.

Part B provides $5 million in supplemental grants to States for Emerging Communities.

For FY 2010 $8.4 million was awarded for MAI activities in 33 States, the District of Columbia , Puerto Rico, and the U.S. Virgin Islands

Of the 2,157 Program providers submitting data for 2008: 1,213 received Part A funds; 1,160 received Part B funds; 444 received Part C funds; and 263 received Part D funds.
