Passport Affidavits (Sworn Statements)

These affidavits are not a required part of an application for a U.S. passport under normal circumstances. The information provided on an affidavit may be used to help establish citizenship, identity, and entitlement to a U.S. passport. These affidavits should not be submitted in place of attainable U.S. citizenship evidence or identification.

Guidelines for Downloading and Printing Documents

The forms you print must be clear and sharp. Forms that are blurred or illegible will not be accepted. Be sure to print your form:

  • Using black ink
  • On 8 inch by 11 inch white paper
  • On one-sided, full pages
  • On matte surface paper (no shiny surface paper)

  • NOTE:
    Please make sure that the photograph area on the printed form remains 2x2 inches in size.
