Health Promotion and Disease Prevention (HPDP) Program What is the Health Promotion and Disease Prevention (HPDP) Program?Many Veterans live with chronic disease or are at risk for a chronic disease. In large part this is due a lack of access to preventive care or to unhealthy lifestyle choices. Recognizing this fact, the Veterans Health Administration (VHA) is shifting its focus from disease management to the overall health and wellness of Veterans. VHA National Center for Health Promotion and Disease Prevention has developed the Health Promotion and Disease Prevention (HPDP) Program specifically to ensure a more holistic, veteran-centered system. The HPDP Program is designed to provide Veterans with comprehensive health education, clinical services and support for self-management of health-related behaviors.
