Learn more about your diagnosis with the My HealtheVet Health Glossary Many Veterans live with chronic disease or are at risk for a chronic disease. In large part this is due a lack of access to preventive care or to unhealthy lifestyle choices. Recognizing this fact, the Veterans Health Administration (VHA) is shifting its focus from disease management to the overall health and wellness of Veterans. VHA National Center for Health Promotion and Disease Prevention has developed the Health Promotion and Disease Prevention (HPDP) Program specifically to ensure a more holistic, veteran-centered system. The HPDP Program is designed to provide Veterans with comprehensive health education, clinical services and support for self-management of health-related behaviors.

The HPDP Program provides evidence-based recommendations focusing on nine Healthy Living Messages. All of the documents below are in PDF format which requires Adobe Acrobat Reader. Each month the HPDP Program will highlight a specific Healthy Living Message, providing education and information on services available throughout the VA New York Harbor Healthcare System.
