Table Template for NIBIB Research Education Programs for Residents and Fellows

The R25 template may be used to provide information on training records of faculty for TPA-11-619. This table should include all trainees sponsored by the listed mentors over the last 5-10 years and the level at which they were sponsored.

Mentor Name

In this column, list individuals who will directly train and mentor participants in the research education program.

Trainee Name

List trainees sponsored by the listed mentors over the last 5-10 years.

Training Position

Specify the title held by the listed trainee while sponsored by the mentor.

Trainee Individual Funding

List individual funding awarded to each trainee while sponsored by the mentor. Exclude from this list any institutional awards to which the trainee was appointed (e.g., T32) or any awards for which the trainee was not the principal investigator.

Subsequent Position

Note each trainee's last known position after leaving the mentor's lab.

Continued in research?

Answer yes, no or unknown. Optional: Provide any additional information you wish to that relates to the trainee's progression in a research career or alternative path.
